This Star of Life Sign designates which elevator can accommodate a stretcher for medical emergency personnel. Typically you will find this sign when the building is four or more stories (above and/or below ground floor level) with one or more elevators for public use.

3002.4a California Building Code: Designation elevators will be identified by the International Star of Life symbol for emergency medical personnel. 30024.5a Symbol Size the symbol will be no less than 3 inches. 30024.6a Symbol Location the symbols will be affixed to both sides of the door jambs. Each symbol should be no less than 78 inches and no more than 84 inches above the floor.
